Informed Consent and Waiver of Liability
 
I desire to participate voluntarily in a training program with Your Live Trainers to try to improve my physical fitness.  I understand that the reaction of my heart, lung, and blood vessel system to such exercise cannot always be predicted with accuracy.  I know there is a risk of certain abnormal changes occurring during or following exercise, which may include abnormalities of blood pressure or heart rate, ineffective functioning of the heart, heart attack or other cardio respiratory medical problems, fainting or death.
  
I understand that the resistance training aspect of my training program is not a personalized program with individual attention; therefore, I (not the trainer) am responsible for monitoring my own condition and exercise form/technique under this program, and I (not the trainer) will use my better judgment in keeping the intensity of the program within the limits of my fitness.  I understand that some exercises may be too intense for me, and I should take breaks throughout the workout as needed.  I shall cease participation should any unusual symptoms occur.  I understand that a program of regular exercise for the heart, lungs, muscles and joints has many associated benefits.  These may include a decrease in body fat, improvement in blood fats and blood pressure, improvement in psychological function, and decrease in risk of heart disease.
 

I understand all of the risks set forth above, and by agreeing to this Informed Consent and Waiver of Liability, I hereby assume all risk for my health and well-being and waive all claims of liability of every type and nature that I may have against Your Live Trainers and its employees/personnel.  I further agree to hold Your Live Trainers and its employees/personnel harmless for any injury or illness I may sustain from my training program or related fitness/wellness activity or component (resistance training, cardiovascular training, nutrition, personal training, and nutrition supplements).  I also acknowledge that I have been informed of the need to obtain a physician’s examination and approval prior to beginning this exercise program.  I also understand that questions about exercise procedures and recommendations are encouraged and welcomed by Your Live Trainers and/or my physician.  (You must be at least 18 years of age or have a parent/legal guardian provide consent to training.)
